The Health Coaching Program in which you are about to enroll in will include all of the following:

A. Two 45-minute appointments each month for six months, which will include a
discussion of your progress, recommendations, and a full set of notes.
B. Monthly special events like teleclasses, group seminars, and/or workshops
related to health and wellness.
C. A variety of handouts, recipes, books, and other materials.

EXPECTATIONS:
During this program, you can expect that I will:
A. Come prepared to every meeting
B. Devote my full attention to you during our time together
C. Offer support, recommendations, feedback, and encouragement throughout this Program
D. Consistently push you out of your comfort zone and try new things

I will expect that you:
A. Comes prepared with bi-weekly worksheets/goals completed to each meeting
B. Show up on time to each meeting, void of distractions
C. Prompt payment for the Program (see Payment section)
D. Ask any questions as they arise
E. Understand that the power of this coaching relationship is ultimately up to you. Commit to it for the entirety of the Program. If problems arise, please communicate that to the Coach.
F. Make this Program a top priority of your life
G. Be open to new ideas and be willing to grow

Our FNPs are trained to provide care for the specialties that are provided at the clinic: pelvic, oncology, lymphatic and vestibular/concussion. Lauren received her Doctor of Physical Therapy degree from Governors State University and a Bachelor Degree in psychology and sociology from Northwestern University. Prior to becoming a physical therapist, Lauren worked as an advocate, mental health practitioner, firefighter and EMT. Lauren’s physical therapy specialty certifications include Certified Lymphedema Therapist (CLT), Pelvic Rehabilitation Professional Certificate (PRPC) and Certified Yoga Therapist (CYT). She is a board member of Pelvic Health Research Initiative and a research member of International Continence Society and North American Menopause Society. As Governors State University faculty, Lauren teaches Special Topics: Pelvic Rehabilitation Physical Therapy, Wellness and Pharmacology. In 2023, Lauren opened Chez Therese to provide pelvic rehabilitation, lymphedema and oncological physical therapy within her community. Lauren treats pregnancy related issues, menopause related issues, pelvic pain, urinary incontinence, bowel dysfunction, sexual dysfunction, lymphedema, lymph node transfer/bypass, stem cell transplant, and bowel/bladder/sexual/functional concerns of patients undergoing HIPEC (hyperthermal intraperitoneal chemotherapy). Lauren has been published in peer reviewed journals within multidisciplinary care and surveillance. Lauren teaches ‘Trauma Informed Care’ to medical providers as faculty of Herman and Wallace Pelvic Institute. As a 2017 Fellow of the Chicago Trauma Collective, Lauren is a trauma-sensitive practitioner whose goal is to empower patients to create meaningful, healthful lifestyle changes to improve their physiology and quality of life.

Allison graduated from Carroll University with a Doctorate in Physical Therapy in 2019. She has experience in inpatient and outpatient settings, specializing in Pelvic Floor Physical Therapy. She has a passion for treating prenatal and postnatal mamas to empower and work alongside them to heal their bodies from the inside out. She has experience in Pilates and loves integrating it into her treatment sessions.
